Filling out the Ukraine Visa Application Form
If you are using the downloadable Ukraine Visa Application form from the Internet and sending to our office for processing please read and follow these simple guidelines:
· ITEMS # 20 THROUGH # 22 of the Ukraine Application Form DO NOT COMPLETE . Ukraine Visa Services processing only.
· Item # 23 - Provide name, address and phone number of contact person in USA.
· Back Page - On the back page, the only thing you need to do is SIGN & DATE at the bottom.
